Call and Respond Early (CARE) – patient, carer activated escalation at Fremantle Hospital
Our CARE program provides a process for patients, families and carers who feel the healthcare team has not fully recognised a patient’s changing health condition.
While our clinical processes can effectively identify deterioration, healthcare teams must also listen to those who know the patient best – relatives, carers and patients themselves.
CARE can be initiated by a patient, relative or carer and results in an independent review by a senior nurse.
The treating nurse or doctor discusses the concerns with the patient, relative or carer, reviews the patient’s documentation and escalates care, if necessary.
If the patient, relative or carer is still concerned and not satisfied with the response a further review will be conducted.
If a patient, relative or carer feels they have not had an appropriate response, they can request a review by an independent responder.
Information brochures about CARE will be provided to patients and carers on admission to hospital.
