Mental Health Service referrals

Referring a patient 18–65 years

The Assessment and Treatment Team (ATT) is the first point of access to Fremantle Hospital Mental Health Service (FHMHS) for adults aged between 18 and 65 who reside in the Fremantle Hospital catchment area. The service links to both inpatient and community mental health services.

The principal function of the ATT is to triage and provide short term care for consumers referred by general practitioners or other service providers. People who present to the mental health service without a referral will be triaged and assessed by ATT before being treated or referred to other services.

Health professionals referring to the community mental health service are asked to complete the relevant referral form (PDF 43KB) or provide appropriate information to facilitate follow up and fax to 9431 3479.

Each referral will be reviewed by the Triage Officer and presented to the next daily ATT intake meeting. The referrer and consumer will be contacted to discuss the outcome and proposed action plan.

Please contact the triage officer by phone between 8am–10pm, 7 days a week to discuss any referrals requiring urgent attention.
