Mental Health Service referrals
Referring a patient 18–65 years
The Assessment and Treatment Team (ATT) Triage Team is the first point of access to Fremantle Hospital Mental Health Service (FHMHS) for adults aged between 18 and 65 who reside in the Fiona Stanley Fremantle Hospitals Group catchment area. The service links to both inpatient and community mental health services.
Referrals received from general practitioners will be triaged and progressed to the Enhanced General Practitioner Liaison (EGPL) team for assessment and management where appropriate.
The principal function of the ATT is to provide short term care for consumers referred by service providers. People who present to the mental health service without a referral will be triaged and assessed by the Triage Team before being treated or referred to other services.
Health professionals referring to the community mental health service are asked to complete the relevant referral form (PDF 43KB) or provide appropriate information to facilitate follow up and fax to 9431 3479.
Each referral will be reviewed by the Triage Officer and presented to the next daily intake meeting. The referrer and consumer will be contacted to discuss the outcome and proposed action plan.
Please contact the triage officer by phone between 8am–10pm, 7 days a week to discuss any referrals requiring urgent attention.

Fremantle Older Adult Mental Health Service
Fremantle Older Adult Mental Health Service (FOAMHS) triage is the point of access to community services for older adults over 65 years who reside in the Fiona Stanley Fremantle Hospitals Group catchment area. The service links with inpatient mental health services.
Health professionals referring to FOAMHS are asked to either:
Each referral will be reviewed by the FOAMHS Triage Officer and presented to the next FOAMHS intake meeting (Monday to Friday). Faxes received after hours will be processed on the next business day. The referrer and consumer will be contacted to discuss the outcome and proposed action plan.
