Ophthalmology referrals
Fremantle Hospital (FH) Ophthalmology Department is the referral centre for all ophthalmic conditions in the South Metropolitan Health Service (SMHS).
It provides more than 13,000 outpatient clinic appointments and 1,300 ophthalmic surgical procedures to the South Metropolitan catchment area. Of this, over half of all ophthalmic surgeries are for cataracts.

Referrals (including from optometrists)
Please include a visual acuity and description of vision-related activity limitations in all referrals.
Referrals from medical practitioners (including general practitioners and ophthalmologists) are to be directed to the Central Referral Service (external site).
Referrals from optometrists are to be faxed to Fremantle Hospital Central Receipting on 9431 2009.
Cataract referrals
Referrers are asked to review the the cataract referral guidelines (PDF 495KB) to provide the ophthalmology department the information required to appropriately and transparently triage patients.
Urgent clinic appointments
For urgent clinic appointments, referring health practitioners are asked to contact the ophthalmology registrar on call by phoning Fremantle Hospital switchboard on 9431 3333.
