Complex Needs Coordination Team (CoNeCT)

The Complex Needs Coordination Team (CoNeCT) is a program which operates across the South Metropolitan Health Service area, with small teams based at Fiona Stanley and Rockingham hospitals.

There is also a North Metropolitan CoNeCT program (contact 0410 666 441).

Patients who are identified by hospital staff as being at risk of further hospitalisation are supported by CoNeCT Care Coordinators to manage their health care at home through chronic condition self management support, linkage to appropriate services and coordination of complex care needs.

Information for GPs

Referral to CoNeCT by GP’s will be considered if the patient has a recent history of frequent Emergency Department presentations which could have been avoided through care coordination intervention in the community.
