Social Work
Social workers at Fremantle Hospital provide a range of services for patients and their families/carers, including but not limited to:
- assistance with planning your discharge from hospital, involving coordination of a range of services, information and necessary referrals
- counselling for health related issues, including adjustment to a new diagnosis, bereavement carer stress and family conflict
- support in accessing community-based care services after your hospital stay
- guidance and support for you and your family in exploring and arranging residential care
- professional assessment and intervention with complex social problems such as elder abuse, family violence, homelessness and substance misuse.
- general information related to community support services, specialist agencies and health and disability support groups
- assistance with legal and financial matters including applications to Centrelink, public housing, Veterans’ Affairs, the State Administrative Tribunal and other services.

The Social Work Department welcomes communication and referrals from GPs for current or booked patients, but regret that we cannot accept referrals for those who are not current inpatients or outpatients.
Social workers in the Department of Community and Geriatric Medicine and Age Care Assessment Teams respond as part of the Aged Care Assessment Team to referrals from GPs for community assessments. A new referral is required for new episodes of service.
